When you successfully submit your registration at the bottom of this form, you will be asked tp pay the $18 minimum, and then have the option to pay the balance now, pay in installments, or bill to your account to pay later.

Weekday Clubs:
Please indicate which clubs your child may be interested in attending so we can better plan. This is an interest inquiry, and you are not making a commitment at this time. Registration for clubs will take place in August 2024 using separate forms.

All information you provide is of high confidentiality and will only be shared with relevant teaching staff, for the sole purpose of serving your child's needs so that their experience in our program is a positive one.

Photo/Video Sharing Permissions:

We like to document the children's experiences in our programs with photos, videos and images of their work. All images shared are appropriate, respectful and carefully selected prior to publishing. Your child and their work will never exhibit name and identifying details, or be published in a way that puts them at risk of safety, embarrassment or other compromised situation of any kind. An explicit permission of the child's parents is sought whenever there is a question.

By completing this form I understand that I consent to:
Sharing unnamed, unlabeled images of my child, my child's art work, project or written material on the Temple website, on Temple' Social Media page or group, and in email communications unless I specifically revoke permissions in the box below.

Bill to your account so you can pay via any method you prefer at your convenience, schedule installment payments, or make an immediate payment via credit card or ACH/eCheck.
